Clinical definitions of nephrotic syndrome

RemissionUrine albumin nil or trace (or proteinuria <4 mg/m2/h) for 3 consecutive early morning specimens.
RelapseUrine albumin 3+ or 4+ (or proteinuria >40 mg/m2/h) for 3 consecutive early morning specimens, after having been in remission previously.
Frequent relapsesTwo or more relapses in initial 6-month period or more than 3 relapses in any 12 months.
Steroid dependenceTwo consecutive relapses when on alternate day steroid therapy or within 14 days of its discontinuation.
Steroid resistanceAbsence of remission despite therapy with daily prednisolone at a dose of 2 mg/kg/d for 4 weeks.
